Hi, I had sent for my new passport even though I won't need it for another 10 months.  Today, at work, I received a call from a very pleasent gentleman at the Seattle Passport office.  He wanted to remind me that many countries will not accept a passport that is within 6 months of expiring.  I told him I was going to Canada and he said that one is not a problem, but anyone going to Bankok or Thailand might have a problem.  I though it was very nice and thoughtfull of him to check with me before he issued the passport. Temporary passports can be obtained from the passport agency, valid for a year.  It takes the documentation on your name change (if any) and the reason for the temporary passport in the corrected gender i.e. .letter from therapist, confirmation paperwork for surgery and the like.  Temporary passports can be extanded once the surgery is complete and Affidavit received by the passport office.  The extended passport will be the same as the temporary one with a statement on one of the back pages that it was extended for the full ten years.  No additional funds or photos are needed for the extention.

Alabama will issue an "amended" birth certificate noting change of name and sex, but will not issue a new birth certificate replacing the original.
Fees: $15.00 for the first copy and $4.00 for each additional copy (fees subject to change).

A surgeons letter who completed your surgery
The next step into the procedure is best handled through an attorney, although at extra cost. You must file a "petition to amend a vital record," state what you want to amend, provide documentation of the reason for your petition, and name the Center for Health Statistics as defendant.
If you live in another state they will accept the court order from that state (according to one source; other sources say non-Alabama residents must file for a court order in Montgomery County, Alabama). The fee for the court order is a separate fee, and varies from one court to another.

DMV The Arizona Motor Vehicles Division requires a letter from a physician stating that the person is currently undergoing hormone therapy in preparation from transition from one gender to another, and that the changes have reached a point where they are "irreversible and permanent." Wording of the letter is critical and must include the words "irreversible" and "permanent."

While Arizona will issue a new driver's license under your new name, and they will change your gender marker provided you have the appropriate documentation, there will be a note in your MVD entry referencing your previous name. This is ostensibly for documentation and traceability purposes, e.g. should someone come looking for "John Doe" they would be able to track back through the computer system and see that name attached to "Jane Doe's" records, or vice versa, to know that "Jane Doe" used to be "John Doe" and that prior records will be found under "John Doe".

According to the MVD, this is not publicly accessible information, but strictly for law enforcement purposes.

-  Secure a copy of DMV Form DL 328, Change of Name and Gender. This is a one-page form that will require both your input as well as input by a licensed California Physician. Generally, this will be your psychiatrist, endocrinologist, or general practitioner.

-  Return to DMV with the form. Upon payment of the $12 fee, a Temporary License will be issued in your new name and gender. A DMV Supervisor will sign the form. We recommend you ask for a copy for your records.
Even though you have a Temporary License at this point, your Permanent License will not be issued until your name on the DMV and Social Security records match and are verified by computer run. The sex flag is no longer compared.

-  As soon as reasonably possible, go to a Social Security office to change your name, and optionally, your sex on its records.
You will need your old Social Security card, proof of your old identity, your temporary drivers license, and proof of your new identity. If you question the sufficiency of the proof of your new identity, bring anything you might have, such as invoices from a health care provider, letters and envelopes addressed to you, or receipts.

In California, the Department of Motor Vehicles DL 328 Form is a confidential medical information authorization form for change of name and gender that can be picked up at any Department of Motor Vehicles office. This form calls for a doctor's signature. Usually the Doctor who is prescribing hormones for you is the doctor who signs this form. So the first step is to pick up the Name and Gender Change form (DL 328) and take it to your Medical Doctor and have it filled out and signed by the doctor. Then return this form to a Department of Motor Vehicles office.

Upon presentation of this form and paying a fee of $12 to the Department of Motor Vehicles your photo will be taken. Therefore it is important to go to the Department of Motor Vehicles in your chosen gender. You will be issued a temporary new Driver's License. If you had previous identification, you will give it to the Department of Motor Vehicles, as it is no longer valid identification. Your new license with your new name, gender, and picture will arrive in the mail within 90 days.

Once the Department of Motor Vehicles processes your Name and Gender Change application, they will place your records in a closed file that is not open to public inspection. In this way they are attempting to protect your privacy. To get a printout of your records for auto insurance or other purposes, you will need to go to a Department of Motor Vehicles office and present your I.D. to a supervisor who will have the password to access your file.

DMV  Colorado does require that formal name changes go through the county clerk and recorder. That's an extra 42 bucks in Douglas County (perhaps more in other counties), plus another 50 to run a legal notice.

The good news is that all it takes is a letter from a therapist, stating that a gender change is appropriate, plus a court order for change of name. To get a new photo, you renew the license; there's another 15 (or 16 if you also have Motorcycle) -- if you're willing to stick with the old photo on a license for the new you, that will only cost 5 dollars, or 10 if you've ever had to have it reissued.

The bad news is that Colorado will keep your old name in the system and it WILL show up on the line 'previous name' in your Motor Vehicle Record. That line shows up on every copy of a Colorado MVR. Therefore, if you ever apply for a driving job (or any other where the employer wants MVR) you're outed whether you like it or not.

DMV Requires a letter from a health care provider attesting that the applicant is in active treatment and is living full time according to the requirements mandated by the Standards of Care for Sexual Reassignment is sufficient documentation to effect the license change.

The wording above is taken verbatim from a memorandum by the Conneticut Legal Services Division Chief, dated October, 1993. Current website info for Conneticut mentions a "court ordered gender change document." It is unclear whether this changes the standards defined above.

Many people who want to legally change their name would have to send their fingerprints to
state law-enforcement agents under a measure a House panel approved Monday 15 March 04.

The bill, written by the House Judiciary Committee and approved unanimously by
the panel, would also require people who petition for name changes to disclose
past arrests and criminal charges, whether they were convicted or not.

Exceptions would exist for women changing their name because of marriage or
divorce, and for adoptions.

The measure is backed by the Florida Department of Law Enforcement, which says
it would get rid of the fingerprints if the person hasn't been arrested, but
would keep them on file otherwise.

We finally did it and I got the note last night.  Birth records in Florida CAN be changed.
Requirements:
"A statement from any licensed physician (that the change was completed or that the original certificate is in fact in error) and a court order for a name change [if desired] and The Office of Vital Statistics will amend the birth certificate.  Effective immediately"

PART II. BIRTH RECORD AFTER CHANGE IN SEX DESIGNATION

§62. Issuance of new birth certificate after anatomical change of sex by surgery

A. Any person born in Louisiana who has sustained sex reassignment or corrective surgery which has changed the anatomical structure of the sex of the individual to that of a sex other than that which appears on the original birth certificate of the individual, may petition a court of competent jurisdiction as provided in this Section to obtain a new certificate of birth.

B. Suits authorized by this Section shall be filed contradictorily against the state registrar in the judicial district court having jurisdiction over the parish in which the petitioner resides or over the parish in which the petitioner was born. A nonresident born in Louisiana shall file the petition in the parish of birth. The suit of any petitioner born in Louisiana shall be filed contradictorily against the state registrar. In the event the petitioner is married, the spouse shall also be a necessary party to the suit. To the extent that the petitioner's name is to be changed, the district attorney shall also be a necessary party. In all cases the petition shall be accompanied by a certified copy of the petitioner's original birth record, in which case the short-form birth certificate card shall not be sufficient.

The court shall require such proof as it deems necessary to be convinced that the petitioner was properly diagnosed as a transsexual or pseudo-hermaphrodite, that sex reassignment or corrective surgery has been properly performed upon the petitioner, and that as a result of such surgery and subsequent medical treatment the anatomical structure of the sex of the petitioner has been changed to a sex other than that which is stated on the original birth certificate of the petitioner.

If the court shall find that the evidence sustains the required proof, the court shall render a judgment ordering the issuance of a new birth certificate changing the sex designated thereon from that shown upon the petitioner's original certificate of birth. The petitioner may in the same suit seek to have the name of the petitioner changed, and the court may render judgment in accordance with law upon this additional petition at the same time.

D.(1) A certified copy of the petition and judgment for a new certificate pursuant to this Section shall be furnished to the state registrar of vital records at New Orleans within ten days after the judgment is rendered. The registrar shall issue to the petitioner a new certificate or certified copy thereof; whereupon the original birth certificate and the copy of the petition and judgment received by the registrar shall be sealed in a package and filed in the archives of the vital records registry.

(2) This sealed package shall be opened only upon demand of the individual to whom the new certificate was issued, and then only by order of the court which rendered the judgment ordering the issuance of the new certificate.

NOTE: Louisiana will NOT change the sex designation on a driver's license unless you can produce a birth certificate with the desired sex designation. So, if you were born in ID, OH, or TN, and live in LA at the time of your SRS, you cannot get your driver's license changed. This is the updated information as of January 2002:

DEFINITION
If an applicant for or a holder of a Louisiana driver's license/identification card indicates that they have undergone a sex change procedure and desires to change the gender identification on the driver's license/ID card, the change will so be noted and a new driver's license/ID card issued.

REQUIREMENTS
The following will be required:

1. A medical statement signed by a physician stating that this person has undergone an operation for a sex change from _______ to ______ and that the sex or gender change has been successfully made.

2. In addition to the above, should the applicant or holder of a Louisiana driver's license/ID card seek a name change, a certified or true copy of a court order must be presented.
NOTE: If the court order specifically requires a corrected/amended birth certificate be obtained, we will require a copy of said corrected/amended birth certificate. If the court order does not specifically require a corrected/amended birth certificate, the court order will be sufficient. However, the applicant/holder of the license/ID card must be advised that the court ordered name change will be the legal name until such time as subsequent court action advises otherwise (At this point, the court order supercedes the birth certificate. He/she can't go back to the original birth certificate name without another court order.)
NOTE: A copy of court order and/or physician's statement must be included in the file.

PROCEDURES
Once the documentation has been verified and all other criteria has been met for the issuance of a driver's license/ID card, CHANGE OF DATA transaction will be processed on the DLRU20.

If the applicant has a Class "E" or "D" license or an ID card, a duplicate license fee will be required. If the license is a CDL license, a CHANGE DATA transaction will be done and the $15.00 application fee will be charged along with the $5.00 duplicate license fee.

DMV  Policy 104.3 Name Change. By law, a Massachusetts resident has the right to change his/her name with or without a court-ordered change of name petition as long as there is no intent to defraud anyone. It took me four attempts at the RMV to get my gender right (after surgery) before they allowed it. Hi there I am writing to you because I used your web info for my own transition efforts and I wanted to let you know so you could post it on the info link that in Michigan it is so easy to get your license done.  All I had to do was to file my name change forms with the court which cost me $150.00, plus some minor fees after then I went to my local Police Department and got finger preinted and sent $53.00 for a processing fee with those prints to the State police, make sure to have them mark the card on back as both state and federal after the state does it search it sends them on to the FBI and I got my report back from the court 5 weeks later.  You will then recieve a notice for your hearing and before you appear you must go to the county court house and file the notice of name change in a legal news nespaper 14 days prior to the hearing, they will send you 6 clippings from the publishings, that cost $58.00  I then broght every bit of paperwork I had filed with me to court, the morning of the hearing you must pay a $10.00 fee with the court for your court order and once the judge grants your name change you then return to the clerks office and pay $11.00 for each certified copy.  Be sure to dress nice and to be very polite and to answer the questions the judge asks you, be short and to the point with your answers.  Unless you are a hardened criminal you should have no difficulties at all, finally once you have your order in hand and paid for all of your certified copies go to the Secretary of State's office and show them your name change order and just tell them you want to change the gender, they will give you a short form to fill out declaring that you wish for your drivers license to reflect your female status and they change it there on the spot and about 10 days later you will recieve your new license in the mail.

Michigan no longer allows "on demand" gender changes on driver licenses.    The Secretary of State requires a letter from a doctor. I had my primary care physician write a letter for me.  It stated that I was a female to male transsexual, and that I had undergone therapy, hrt, and surgery.  He did not state what type of surgery.

A person who wishes to change the gender indicated on his driver's license, motorcycle driver's license or identification card must submit proof satisfactory to the department that his gender was changed. The proof must include documentation prepared by a physician or an osteopathic physician indicating that the gender of the person has been changed.
A statement by a physician who practices in the area of psychiatry that the gender of the person will be changed or is in the process of being changed is not sufficient documentation for the purposes of subsection 1.
If a person requests that the department change the name and the gender indicated on his driver's license, motorcycle driver's license or identification card, he must submit proof that his name was legally changed as prescribed in NAC 483.055 in addition to the documentation set forth in subsection 1.
As used in this section:
(a) "Osteopathic physician" means a person who is licensed to practice osteopathic medicine pursuant to chapter 633 of NRS or the laws of another state or jurisdiction.
(b) "Physician" means a person who is licensed to practice medicine pursuant to chapter 630 of NRS or the laws of another state or jurisdiction.

Web Submission Jun 2006:  I have an update, based on recent experience, to the information on changing your birth certificate in SC.

To change your name and gender on your South Carolina birth certificate, you will need certified copies of court order(s).  The South Carolina Department of Health and Environmental Control will not accept a letter from your surgeon in order to change your gender marker.  DHEC does not accept photocopies of court orders, or common-law name changes.

It's highly recommended you hire an attorney to help you with all this, since many family court judges have never done this and, unlike name changes, there's no specific statute for gender change.  It has been done before, though, so recent precedent exists.  An attorney is a very good investment, since he or she can talk to the judge and attempt to educate him or her.

If you're successful, you can then apply to DHEC for the changes with certified copies of your court order(s).  DHEC will issue an amended birth certificate; your old name won't be given on your new certificate, but there will be comment lines that state what has been changed, and when.  Not perfect, but better than what some states will give you.  Be polite to the DHEC folks; I was treated with kindness and professionalism when I went to apply for my changes, so be good to them and they'll probably be good to you.     Good luck!    Jenny

Tennessee is one of three states that refuses to allow transgendered persons to correct the sex designation on their birth certificates and the only state with a statute that specifically prohibits the correction of sex designation on a birth certificate.

The Senate General Welfare Committee approved a bill Wednesday sponsored by Sen. Steve Cohen (D-Memphis) that would allow people with a sex change to correct the sex designation on their birth certificates when showing a sworn statement by a licensed physician that indicates the gender of the individual born in Tennessee was changed by surgical procedure.

DMV In the Lone Star State, you need a court order from a judge to get the gender marker changed. That's done when you file a petition to get your name changed at the same time.

If you are successful, and that depends on where in the state you live, the court order is then taken to any Texas Department of Public Safety (our state troopers) office where you'll be issued a new license.

Unfortunately, the GOP in Texas was one of the first parties taken over by the Reich in 1994, and if you get a GOP judge, they will do one of four things:

They will deny the petition totally.
They will deny the petition and disrespect you at the same time.
Grant the name change, but not the gender code change
Make you add a line to you petition that states that you can never get married to the opposite sex before they will grant the name and gender change.

Virginia will issue an amended birth certificate (except as noted in the law below).
§ 32.1-269. Amending vital records; change of name; acknowledgment of paternity; change of sex.

A. A vital record registered under this chapter may be amended only in accordance with this article and such regulations as may be adopted by the Board to protect the integrity and accuracy of such vital records. Such regulations shall specify the minimum evidence required for a change in any such vital record.

B. Except in the case of an amendment provided for in subsection D, a vital record that is amended under this section shall be marked "amended" and the date of amendment and a summary description of the evidence submitted in support of the amendment shall be endorsed on or made a part of the vital record. The Board shall prescribe by regulation the conditions under which omissions or errors on certificates, including designation of sex, may be corrected within one year after the date of the event without the certificate being marked amended. In a case of hermaphroditism or pseudo-hermaphroditism, the certificate of birth may be corrected at any time without being considered as amended upon presentation to the State Registrar of such medical evidence as the Board may require by regulation.

C. Upon receipt of a certified copy of a court order changing the name of a person as listed in a vital record and upon request of such person or his parent, guardian, or legal representative or the registrant, the State Registrar shall amend such vital records to reflect the new name.

D. Upon written request of both parents and receipt of a sworn acknowledgment of paternity executed subsequent to the birth and signed by both parents of a child born out of wedlock, the State Registrar shall amend the certificate of birth to show such paternity if paternity is not shown on the birth certificate. Upon request of the parents, the surname of the child shall be changed on the certificate to that of the father.

E. Upon receipt of a certified copy of an order of a court of competent jurisdiction indicating that the sex of an individual has been changed by medical procedure and upon request of such person, the State Registrar shall amend such person's certificate of birth to show the change of sex and, if a certified copy of a court order changing the person's name is submitted, to show a new name.

F. When an applicant does not submit the minimum documentation required by regulation to amend a vital record or when the State Registrar finds reason to question the validity or sufficiency of the evidence, the vital record shall not be amended and he shall so advise the applicant. An aggrieved applicant may petition the circuit court of the county or city in which he resides or the Circuit Court of the City of Richmond, Division I, for an order compelling the State Registrar to amend the vital record; an aggrieved applicant who was born in Virginia, but is currently residing out of State, may petition any circuit court in the Commonwealth for such an order. The State Registrar or his authorized representative may appear and testify in such proceeding.

  • A letter from the applicant explaining the change being requested.
  • A letter from the applicant's physician verifying that the applicant is in the process of, or has completed, a gender change.

Wisconsin will change both name and sex, and will issue a new birth certificate. NOTE: You must check the box on the application form to "IMPOUND" the original. This causes the original certificate to be closed to further inspection.

Thank you for contacting WisDOT for information on how gender discrepancies between the driver record and the Social Security
Administration (SSA) record will be handled with the implementation of  SSOLV.

The SSA record match compares the name, date of birth, social security number and gender, with the driver license record and identifies any  driver with a record that does not match the SSA name, date of birth, social security number and gender exactly. Only about 2% of the WisDOT records have any type of discrepancy as a result of the match with SSA records.

Wisconsin has always required an affidavit or statement that the driver is enrolled in a program leading to gender change, or has completed the surgery prior to changing the gender designation on the driver license.  The statement must be from a medical doctor or director of a facility specializing in gender change. This requirement is not new and will not change with the implementation of the Social Security Online Verification system (SSOLV). Drivers who provide appropriate physician documentation will be allowed to change the gender designation on their driver license one time. This will allow the driver time to complete the procedure and obtain a new Social Security Card. The WisDOT record will be notated and no future driver license will be issued until the SSA record is updated to reflect the new gender designation.

Wisconsin has never allowed a person to change his/her gender designation on the driver license until the applicant is enrolled in a gender change program or has completed the gender change surgery. All drivers must designate their physical birth gender on the application, to obtain a driver license.  The gender designation can be changed only if the driver provides physician documentation that he/she is enrolled in a gender change program or has completed the surgery.

The driver license must designate the physical gender of the driver, while the photo may reflect the appearance of a different gender. Although it may be inconvenient for a driver, if the physical gender and photo on the driver license do not match, it does not impact the driver's ability to obtain a driver license or to use that driver license for identification purposes.

Implementation of SSOLV did result in WisDOT changing its name change policy. Effective July 5, 2005, documentation (such as court order, marriage certificate, divorce decree, etc.) will be required to change the name on the driver license. When there is a name change, drivers need to:
1)  take the appropriate documents to the SSA to record the new name and
2) bring appropriate documentation of the name change and new social security card to DMV for a driver license in the new name. For more information regarding SSA acceptable proof of identify documents or SSA locations see the SSA web site at:  www.social security.gov. The following information is current as of March 2002, according to my sources:
I live in New Brunswick, Canada, I was born in Nova Scotia, Canada.
First I had to visit the Vital Statistics New Brunswick office on-line to change my name:

They have all the info and you can order the 'kit' on-line. I was asked to provide a letter from a therapist as to verify the nature of my request. I also had to provide an original 'long-form' birth certificate, because I was born out of province. I ordered that on-line from this site:

It cost $25 and was delivered promptly.
After all that, and $100 later to PNB, I got my 'Change of Name' certificate.
Then you go back to the PNS (Province of Nova Scotia) site. In order to change name and sex, you will need to supply this Change of Name certificate, a letter from your SRS surgeon, and a letter from another physician who has examined you post op.


Ontario
Ontario will issue an amended certificate, changing either name or sex, and the certificate will not reveal which items were changed. The change of the sex designation on the birth registry is a separate process from the change of name. A copy of the long form of the certificate will list the original name and sex, along with the new name and new sex, plus annotations describing the changes made.
Change of Name: Contact the Registrar General's office and request an Application to change an adult's name. You will need original copy of your birth certificate, a signature of a guarantor, and a signature of a commissioner. Present cost is C$137.
Change of Sex Designation: Contact the Registrar General's office and request an Application for Change of Sex Designation on Birth Registry package. There are three forms to be filled in. You fill in the Application for Change of Sex Designation on Birth Registry form. The surgeon who conducted the transsexual surgery the Medical Certificate of Transsexual Surgery. And another physician completes the Medical Certificate to Substantiate Transsexual Surgery was Performed form. The cost to have the change performed is C$37. Cost of the short form (certificate) is $C15 while the cost of the long form is C$22.

Changes Resulting from Transsexual Surgery

Changing sex designation appearing on registration of birth

Where the anatomical sex structure of a person is changed to a sex other than that which appears on the registration of birth, the person may apply to the Registrar General to have the designation of sex on the registration of birth changed so that the designation will be consistent with the results of the transsexual surgery.

Application

An application shall be accompanied by,

(a) a certificate signed by a medical practitioner legally qualified to practise medicine in the jurisdiction in which the transsexual surgery was performed upon the applicant, certifying that,

(i) he or she performed transsexual surgery on the applicant, and

(ii) as a result of the transsexual surgery, the designation of sex of the applicant should be
changed on the registration of birth of the applicant;

(b) a certificate of a medical practitioner who did not perform the transsexual surgery but who is qualified and licensed to practise medicine in Canada certifying that,

(i) he or she has examined the applicant,

(ii) the results of the examination substantiate that transsexual surgery was performed upon the
applicant, and

(iii) as a result of the transsexual surgery, the description of the sex of the applicant should be
changed on the registration of birth of the applicant; and

(c) evidence satisfactory to the Registrar General as to the identity of the applicant.

Alternate medical evidence

Where it is not possible to obtain the medical certificate referred to in clause (2) (a) or (b), the applicant shall submit such medical evidence of the transsexual surgery as the Registrar General considers necessary.

Notation on birth registration to be consistent with result of surgery

The Registrar General shall, upon application made to him or her in accordance with this section, cause a notation to be made on the birth registration of the applicant so that the registration is consistent with the results of the surgery.

Old certificates to be returned

Any person in possession or control of a certificate or certified copy of a birth registration issued before the making of a notation under subsection (4) shall return the certificate or certified copy to the Registrar General forthwith upon demand.

You can change your passport before you change your birth certificate, if you wish.  That is found in the Federal regulations.  Not as straightforward and you will still need the birth certificate at a later date for yoru state authorities (i.e. driver's license).

From Stephen   Applying for a change to your UK Birth certificate:

1. England or Wales.
Write to General Register Office,
Corrections and Re-registration section

2. state your full new name, old name, date of birth and place of birth
and (if you have it) the number of your birth certificate (send a photocopy if you can)

3. State what date you commenced living full time in your new gender.

4. State you have undergone gender reassignment treatment, and that you wish to apply for a change to your birth certificate in the light of the ruling of the ECHR in Goodwin & I v UK Government.

Please DO NOT indicate your operative status or what surgery you have had!

NOTE: it is important for the rights of all/any T*-people who cannot have full SRS surgery, or don´t want all forms of treatment, that the Registrar General does not get a feeling she can decide to grant B/C´s only to any T*-people who have had full SRS... The aims of PFC are clear, that any treatment which is considered to be part of your permanent modification of your body is a full valid legal indicator for the right to request a new Birth Certificate. This is especially true to ensure the rights of f2m (Transmen) who cannot yet get what can be described as safe and satisfactory phalus-construction surgery.

5. Include your statutory declaration of name change and your doctors letter indicating that the change Petra´s note: Even if is just Breast growth through HRT) has been PERMANENT

6. Copy all the correspondance to your Member of Parliament, at the House of Commons, London WC1A 1AA

7. Include a cover note to your MP asking that they chase up this matter and ensure it is expedited in view of the long time you have had to wait to date for this.

I think the Registrar General will write back saying they cannot do this until english statutory law is changed - we might then have to have a class action seeking judicial review of the refusal to do this.

But in order to get that we need as many of you as possible to apply NOW

8. If you are married, do not mention it on the request, then the issue of requirements for a divorce to get a legal sex-change is not only not under consideration, but is irrelevant for the decision. It has already been allowed, as was granted in my own case (where I am now 100% legally female and can only marry a man) but was told the exisiting marriage is and remains untouched. So if you later have difficulties due to this, write to me and I will send you copies of the various
documents and info you can cite!

Support this action and make use of the rights fought for and granted by the decision of the UCHR in July 2002

Note: I  just found out that Illinois has changed their rules about birth certificate changes, to require that both the patient and the surgeon submit forms certifying that "The surgery has been completely performed" and now recognizes ONLY CERTIFICATES SIGNED BY PHYSICIANS LOCATED IN THE UNITED STATES!  Robyn
I believe that even if done overseas your local GP attesting to the fact that it has been completed would meet the requirement.

You are able to do Comon Law Name Changes in most states and it may be something that you would like to consider.  Below is a note from one successful changed done this way.

Hi Dalelynn
Just a quick note..  I did a document called "AN AFFIDAVITE AND DECLARATION OF COMMON LAW NAME CHANGE". It was prepared by an attorney friend of mine.. All I did was fill in the blanks spaces and have it notarized. It stated who I was.. and who I plan to be.. living as a female.  Using that document, I went to the social security orifice and stood in line for about an hour. I filled out a name change form and handed in my old card the name change form..a letter from my therapist explaining my status as GID in transition and living my RLT,  and the name change declaration.  The nice lady behind the counter also changed the gender part of my record (something not always
done).

About two weeks later upon recieving my new card (with the same old number on it) I went to DMV.  I filled out the change of name forms, an origional of my therapists letter (they made me a copy to keep) and showed my new S.S. card. They took my money and new photos and gave me my new license. The supervisor who took care of me smiled as she handed my my license and said "welcome to womanhood". and that was that.. two weeks later the photo license showed up in the mail and I was done.

DMV UPDATE:
Because of September 11, (911) many DMVs have instituted the requirement for additional identification as proof of legal presence.  Most states that have done so require this upon initial request, renewal of an expired license or reinstatement of a suspended license.  This also applies to Photo ID Cards.
Legal presence means that a person is either a U.S. citizen or is legally authorized to be in the United States. Legal presence can be proved using a U.S. birth certificate or U.S. passport. It also can be proved using a variety of other documents such as a Certificate of Citizenship or Naturalization, Resident Alien Card or a valid foreign passport with a visa, I-94 or an I-94W with a participating country.
See your local DMV for specific form or documents required as proof of legal presence.
For Virginia the Birth record, U.S. passport, Certificate of Citizenship or Naturalization, Resident Alien Card, or a valid foreign passport with a visa, I-94 or and I-94W with a participating country are valid.
